Tuesday, October 2, 7:00 a.m. - 9:00 a.m. (Check-in at 6:30 a.m.)
Pre-Application Only!  Only 60 places are available for testing, so don’t delay.  Test sessions will be filled on a first in-first confirmed basis.

Earn your CHESP credential during ASHES’ Annual Conference.  The CHESP Examination will be administered during the above times and will be administered on laptop computers.  Candidates will receive scores after the Examination.  Laptops will be provided.

1. Download and print the 2007 CHESP Application Form here or by visit:  http://www.aha.org/aha/Certification-Center/CHESP/index.html  or call Applied Measurement Professionals at 913-895-4600 for a hard copy of the handbook.
2. Review the requirements, regulations and information on the exam.
3. Complete the above application or complete pages 17 and 18 of the handbook.  IMPORTANT: On the application, under “Examination Type,” check “special administration” and enter Monday October 1-2 as the scheduled date of administration.
4. Remember to sign the application.
5. Fax with a credit card or mail both pages (17 and 18) with payment to AMP.  Mail to the address provided on the application or fax to 913-895-4650. 

Applications must be received by August 27, 2007 to take the exam at the annual conference.  On-line application is not available for this administration.  AMP will fill the first session first and then fill the second session as applications are received.  You will be notified as to what session to sit for the exam.

Important Reminders – PLEASE READ:
- Regulations in the CHESP Candidate Handbook apply to all applications.
- The Examination fee may be paid by credit card, money order or company check (No personal checks).
- Applied Measurement Professionals (AMP) will process applications and send confirmations 3 weeks in advance of the test date.  You will not receive a confirmation before this time. If you do not receive a confirmation by September 7, 2007, please contact AMP directly at 913-895-4600.
- There is no online application for the Exam.  Candidates must complete the application process as outlined above.
- A review session will be held on Sunday, September 30th at the America’s Center from 3pm – 5pm.  Review session registration is separate from the exam application.  A review session registration does NOT constitute an Exam application. Applicants can sign up for the Review Session online or through the conference brochure.
